How to make a daycare sign in and out sheet
-
Add your daycare’s name and the date
Type your name or logo at the top. They print on every page. Leave the date empty if you want a line to write it by hand each day.
-
Check the columns
The sheet starts with the child’s name, the parent or guardian, time in, signature in, time out and signature out. Rename, widen or remove any column.
-
Print a stack
Type the children’s names into the names box so they print in the first column, or leave it empty for blank rows. Rows are tall enough to sign in. Print one page per day.
What to put on a daycare sheet
- The child’s name, so the right row is signed
- The parent or guardian who brings the child, and who collects them
- Time in and time out, written by hand or by the parent
- A signature at drop-off and another at pick-up
- The date at the top of the page, and your daycare’s name
Ask only for what you need
Everyone who signs can read the rows above theirs, so keep health details, allergies and anything private off this sheet. Keep those on a separate form that stays with you. This is not legal advice: what a child care provider must record and how long to keep it depends on your state or country, so check your state’s rules.
Tips
- Print a fresh page each morning and keep it on a clipboard by the door, with a pen on a string.
- If more than one adult collects children, add a column for “Picked up by”.
- At closing time, check that every child who is signed in has been signed out.
- File the finished pages by date in a folder, away from where parents can read them.

Frequently asked questions
Where can I get a daycare sign in sheet template?
Make one here. Pick the columns you need, add your daycare’s name, and print the page or save it as a PDF. It is free and needs no account.
What is a child care sign out sheet?
It is the part of the sheet where the parent or guardian writes the time and signs when they collect the child. This sheet has a time out and a signature out column next to the sign-in ones.
Can I print the children’s names in advance?
Yes. Type one name per line in the names box and they print in the first column, one per row, so parents only add times and signatures.
Should I put allergies or medical notes on the sheet?
No. Others can read the page as they sign. Keep that information on a separate private form.
How long do I keep the sheets?
That depends on your state or country and any licence you hold, so check your state’s rules or ask your licensing office.
Is this daycare sign in sheet free?
Yes. It needs no account and adds no watermark. What you type stays in your browser and is never uploaded.
