Two per sheet saves paper: the questions go into two columns and you cut along the dashed line. The Simple design prints in black and grey only.

Wording: change the title and the other fixed words

Each box shows the words printed now. Rewrite any of them; an empty box goes back to the standard wording. You can also click the words on a form to rewrite them.

What is a Secret Santa questionnaire?

A Secret Santa questionnaire is a short form each person fills in so their gift giver knows what they like. It asks about favorite colors, snacks, hobbies, sizes and things to avoid. Print one for every person in the group and collect them before names are drawn.

How to make Secret Santa forms in 3 steps

    1. Add the exchange details

Type the event or group, the exchange date, the spending limit, and who should get the forms back and by when. Anything you leave empty prints as a line to fill in by hand.

    2. Add the names and pick the questions

Paste one name per line and each person gets a form with their name on it, or leave the list empty for one blank form to copy. Keep the standard questions or rewrite, remove and add your own.

    3. Print or share the PDF

Print one form per page, or two per page to cut in half. Or share the PDF in your group chat so everyone prints and fills in their own.

How to run a Secret Santa

    1. Pick a date for the exchange and agree on a spending limit that suits everyone.
    2. Hand out the questionnaires a week or two before the draw, with a date to return them.
    3. Collect the forms, then draw names: each person takes one name out of a hat (and draws again if it is their own).
    4. Give each Secret Santa the form of the person they drew, and keep the matches to yourself.
    5. On the day, put the gifts in one place and hand them out, or let each Santa deliver theirs.

Pick the set that fits your group. In the form, remove the questions you do not need, then use “+ Add a question” to type these in.

    Questions for coworkers

Keep it light and work-safe, and leave out anything too personal.

    • How do you take your coffee or tea?
    • Favorite snack to keep at your desk
    • Something that would make your workspace nicer

    • Hobbies outside work
    • Do you prefer funny or practical gifts?
    • Any allergies, or things you cannot eat?

    Questions for family

Families often set a higher limit, so a wish list and sizes help most.

    • Favorite color and scent
    • Something you have been meaning to buy for yourself
    • Books, movies or shows you love

    • Your go-to drink order
    • A guilty pleasure snack
    • Dream vacation spot
    • A song or artist on repeat
    • Something that always makes you laugh
    • Favorite way to relax
    • A small luxury you never buy yourself
    • Gag gift or real gift?

    Tips for a fun exchange

    • Keep the spending limit the same for everyone, and say whether it includes gift wrap.
    • Ask about allergies and things people cannot have, so food and scented gifts are safe.
    • Questions are optional: tell people to skip any they would rather not answer.
    • For a remote team, add a question for a mailing address, or agree to swap e-gift cards.
    • Two forms per sheet saves paper. Cut along the dashed line before you hand them out.
    • Keep the forms somewhere private after the exchange, or give them back to their owners.

    Frequently asked questions

Is this Secret Santa questionnaire free?

Yes. It is free to use, needs no account or sign-up, and never adds a watermark or a SnapPaperwork logo to your forms.

Can I make a form for everyone in the group at once?

Yes. Type or paste the names, one per line, into “Who gets a form”. Each person gets a form with their name filled in, in one print or one PDF. Up to 60 at a time.

    Can I change the questions?

Yes. Rewrite any question, remove the ones you do not want, add your own (up to 20) and choose how many lines each one gets. Click a question on the preview to rewrite it there too.

Can I fill it in on my phone or computer?

Yes. Type your answers next to each question and they are printed on the lines, then download or share the PDF with your organizer. Leave the answers empty to print blank forms for a group.

Can I print two on one page?

Yes. Choose 2 forms per sheet: the questions are set in two columns and the page has a dashed line to cut along. With many questions, one per sheet gives more room to write.

Is my information uploaded or stored on your servers?

No. Everything you type stays in your browser and is never sent to me. The form is saved on your own device so it is there next time.

What questions should I ask for Secret Santa?

Ask about favorite colors, snacks and stores, hobbies, sizes, allergies and things people do not want, plus a short wish list. The form starts with fourteen questions like these. For coworkers keep them light and work-safe; for kids use short questions they can answer themselves.

What should I ask a coworker for Secret Santa?

Good work-safe questions are their coffee or tea order, a favorite desk snack, a store or café they like, their hobbies, whether they prefer funny or practical gifts, any allergies, and one thing they would rather not get.

Can people fill in the questionnaire on screen instead of on paper?

Yes. Type the answers into the form or straight onto the page, then save it as a PDF and send it to the organizer. Answers are only saved in your own browser.

What is a good Secret Santa spending limit?

Many offices choose around $20 to $30, and families often go higher. The best limit is one everyone in the group is comfortable with, agreed before the names are drawn.
